Step 1 – Your healthcare provider such as your GP, social prescriber, psychiatrist, social worker, support worker or similar will talk about the programme with you and provide you with a link to the registration form. You can also self-refer by completing the registration form.
Step 2 – Once we have received your registration form, Angela will contact you and arrange for you to meet with her to discuss the programme and may ask you to provide further details.
Step 3 – If you wish to attend, we will offer you a place subject to space being available and notify you of the offer.
